Aifa, in 2025 about 18 packs of medicines on average to citizen: spent 39.3 billion

ROMA (ITALPRESS) – In 2025, “we have been consumed on an average basis, under agreed assistance, about 18 packs for each citizen and 1,148.2 doses per thousand inhabitants every day (+1% compared to 2024)”. Considering the entire territorial, public and private assistance, “the dispensed packages were about 1.9 billion, slightly reduced compared to the previous year (-0.5%)”. Approximately 7 out of 10 citizens (66.9% of the assisted) received at least one prescription of drugs. Some of the main elements emerge from the OsMed 2025 Report on the use of medicines in Italy, published by the Italian Pharmaco Agency on the institutional portal. The total pharmaceutical spending reached 39.3 billion euros, with an increase of 6% compared to the previous year. Public expenditure was EUR 28.4 billion (+5.8% compared to 2024), 72.2% of total pharmaceutical expenditure and 20% of total public health expenditure. Pharmaceutical spending on purchases by public health facilities (including hospitals, ASL, RSA, penitentiary structures and direct distribution channels and on behalf) amounted to 18.9 billion euros, up 6.2% compared to 2024. Public territorial expenditure (including expenditure on Class A drugs provided under agreed assistance, direct distribution and on behalf) amounted to EUR 14.3 billion, (+3.5% compared to 2024).

The expense of the citizens has been pairs to 10,7 billion euros, with an increment of 7.5%, drawn from the purchase of the drugs class A purchased privately (+16.2%), of the drugs of band C with recipe (+7.4%) and of those of self-medication (+6.5%). In 2025, tirzepatide appeared first among the A-range drugs purchased privately by the citizen: in particular, with 225 million euros, it affects for 13% on the private expenditure of the A-class drugs. Compartecipation expenditure, supported by citizens for the quota exceeding the reference price of patented drugs, was 1,04 billion euros, equal to 17,71 euros per capita. It can be noted that in the less-income regions, such as Calabria, Sicily and Campania, citizens pay their own pocket the highest figures (22.35 euros per head against 13.73 euros in the North), also indicates a resistance to replacing the brand drug with the generic. Per capita expenditure and consumption grow with age increase, in particular the population with more than 64 years absorbs more than 60% of expenditure and almost 70% of doses. The Northern regions have a lower prevalence (63.5%) than those of the Centre (68.9%) and the South (70.4%).

“The OsMed Report continues to represent an extraordinary tool to understand the evolution of pharmaceutical assistance in our country – says Technical-scientific Director Pierluigi Russo –. The 2025 edition highlights how the dynamics of spending are closely intertwined with demographic changes, the introduction of new therapeutic options, prescriptive models and territorial differences in consumption. The availability of more and more complete data allows you to describe and understand these phenomena, but also to guide the policies of the drug, promote the appropriateness prescribed and assess the impact of the strategies adopted.” The 2025 edition analyzes the main national phenomena: among the novelties stand out the deepening on drugs for respiratory disorders in pediatrics, the analysis of new high prescription therapeutic categories and the expansion of the evaluation of regional variability. In a second phase, specific thematic and territorial insights will be elaborated, designed to represent with greater detail the complexity of pharmaceutical assistance.

In this edition of the OsMed Report, moreover, in the analysis of the private purchase of class A drugs for the period 2023-2025 the data of sell-out, coming from the dateset Federfarma, Assofarm, Farmacie Unite and Promofarma, processed by Pharma Data Factory and subsequently analyzed and integrated by AIFA with the necessary information sources. The new approach allows to measure the actual dispensations at the expense of citizens and to overcome the criticalities of saddle-in data, influenced by the phenomena of accumulation and disposal of stocks in pharmacies.
