PALERMO (ITALPRESS) – Endometriosis is a chronic, inflammatory and progressive pathology that affects a significant proportion of women at reproductive age, with a significant impact on the quality of life, fertility and social and health costs many of which often have to turn to assisted medical procreation to have children. Diagnostic delay, often of many years, the complexity of the clinical picture, the variability of manifestations and the need for a multidisciplinary approach have made the definition of a Assistential Therapeutic Diagnostic Path (PDTA) structured and shared.
In Sicily this was established with the Endometriosis Network Sicily (Regional Law no. 27 of 2019) to ensure a multidisciplinary and uniform approach to pathology and was officially presented last month. The regional network is based on two main reference centres: Arnas Civico – Di Cristina di Palermo, seat of the Complex Operating Unit of Gynaecology and Ostetricia directed by Antonio Maiorana, identified as the coordinator for the taking and follow-up of patients, and the Arnas Garibaldi of Catania, with the Materno-Infantile Department directed by Giuseppe Ettore, clinical reference point and surgical treatment of complex and territorial cases. The PDTA aims to overcome diagnostic delays and provide integrated and continuous care throughout the region.
But there is still work to do involving the different components of the health system. This was discussed this morning in the first part of the scientific conference “La Pma del III millennium” at the Marina Convention Center in Palermo. A scientific meeting organized by the Cbr, the Centre of Biology of reproduction founded by Ettore Cittadini (which opened the works with a magisterial reading) and by the study of the doctor Andrea Biondo who was introduced by the greetings of the mayor Roberto Lagalla and the deputy mayor Giampiero Cannella and the regional Councillor for transport, Alessandro Aricò. “Scientific research on assisted procreation has made important steps ahead, thanks to a multidisciplinary and integrated approach that today is at the heart of this significant conference, a precious opportunity to take stock of the latest innovations in the field of PMA. In light of the worrying rate of denatility in Italy, it is evident that studies and new techniques can offer concrete support to many women and young couples, often forced to face complex and long paths to be able to have a child,” the mayor recalled in his greeting. It starts from the Pdta network which is based on the two Sicilian centers but which is still to be built.
“In fact, in addition to the two centers, we will have to build the network and understand in all the other provinces which are the centers that can have the characteristics to fit into the same”, explained Ettore present at the meeting “the network that arises from a political project and that we want to extend within the region wants to bring the principle of equity all women must have the same opportunity to access. It is necessary to make a cognitive survey of all structures to understand who does what and put them on the net.”.
For Antonio Maiorana “endometriosi is a complex welfare area because it depends on the characteristics that the disease itself has; it serves a multidisciplinary and capillary approach in the territory. Only with a support network project that has the characteristics of proximity and the possibility of vehicular depending on the intensity of care,” he explained.
From the morning it emerged that the model is similar to that of the oncological network “where, however, the guidelines are much more standardized and more net, but in the endometriosis disease some therapeutic attitudes are still unclear from the scientific point of view and lends itself to a huge variation of theerapeutical possibilities that often confuse the patient”, warned Maiorana.
“The approach to this disease,” he added, “often we did what we can do best, in reality we have to do what we need to do by addressing the patient where there are the best care for his case.” For Antonio Simone Laganà, new director of the specialization school of the Gynaecology School of the University of Palermo: “These days are very useful for our residents to understand what to do and what not to do in the management of infertile couples when there are diseases like that of endometriosis; If we can’t cure women well, we lose the most important part of our society, training future professionals to a management of inferior couples is our mandate.”.
“I had the honour to the presence of my teacher Ettore Cittadini to open the work on this delicate but exciting theme for the new challenges of modern medicine”, explained Andrea Biondo, owner of the homonymous study that organized the meeting and scientific manager of the day “we talked about the topic in a multidisciplinary way touching the various points and above all reasserting that in addition to scientific progress and necessary an ever more attentive humanized and personalized treatment for each couple”.
Giovanni Ruvolo, embryologist of the Cbr and scientific manager of this morning’s conference “journ as these are fundamental to build territorial networks. Those who deal with endometriosis know that they must have a close relationship with medically assisted procreation centers. At this stage of structuring one must understand how these centres must be integrated. What emerged that before you diagnose before you can preserve the fertility of women. The timeliness of the diagnostic phase is fundamental. For the patient it is important to have paths and as clear as possible and defined as possible. This is the opportunity to make Sicily avant-garde at the national level.”.
Antonio Venezia, professor at the University of Palermo and director of the ostetric clinic of the same, has moderated the morning work and is one of the students of the school of Professor Ettore Cittadini “The Professor Cittadini has been able to optimize our path.
